To request an urgent appointment for Monday to Friday
· Use an online form between 07:00am to 13:30pm Monday to Friday to request an appointment. We will try to respond within 3 hours when we are open. We are open Monday to Friday, 08:00am to 18:30pm

· Visit the surgery and speak with a receptionist. We are open Monday to Friday, 08:00am to 18:30pm
When you get in touch, we’ll ask what you need help with. We will use the information you give us to choose the most suitable health professional to help you.
Please note that our eConsults are subject to capacity limitations and will be turned off if required. We will re direct you to 111 if needing urgent advice that doesn’t warrant 999. We will still answer all calls regarding appointments.

Please note: If you have a form that needs to be completed by a GP or requesting a private letter please inform reception, you may not need to book an appointment. This is a non NHS service that may incur a charge which must be paid prior to completion of the form/ letter. We will contact you if an appointment is required.
If you are more than 10 minutes late it is up to the clinician to decide if they are still able to see you.
Your appointment
We may then offer you a consultation:
· By phone
· Face to face at the surgery
· On a video call
· By text
Appointments by phone, video call or by text can be more flexible. This often means the doctor or nurse can help you sooner. We are unable to provide you an approximate time of their call but we will provide a timeframe.
Cancelling or changing an appointment
If you can not make your appointment let us know as soon as you can so another patient can book the appointment time. You can cancel or change your appointment:

If you need help when we are closed
· Use NHS 111 online (111.nhs.uk) or call 111. 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
· Evening and weekend appointments are available from 6.00pm to 8.00pm Monday to Friday, 8.00am to 4.00pm Saturdays and Sundays 10.00am to 2.00pm. Appointments are at the local Health and Wellbeing Centre.
· If it’s a life-threatening emergency call 999.
If you need help with your appointment
Please tell us:
· If there’s a specific doctor, nurse or other health professional you would prefer to see
· If you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text
· If you need an interpreter
· If you have any other access or communication needs.
Home Visits
To request a home visit contact the surgery before 10.30am.
Remember that we can see several patients at the practice in the time it takes for one home visit. Patients should have their appointment at the surgery if they can.
Lack of transport is not a reason to request a home visit.
